SMART Program
Our Programs
Established in late 2008, CCHN's SMART Program seeks to enroll clients already eligible for Federal disability coverage or other health coverage into the appropriate programs and assist them with accessing healthcare services in the community. Our goal is to help them find a medical home outside of area Emergency
Rooms, which have served many of them as their primary source of medical care.
The SMART Program is a community collaborative involving a number of healthcare providers, government agencies, and social service providers. In addition to the community clinics which are CCHN members, the program participants include the Sacramento County Department of Human Assistance, the Mental Health Division of the County Department of Health & Human Services, the regional Social Security Administration office, staff of the County's Ten Year Plan to End Chronic Homelessness, the regional hospital systems, The Effort Medical Clinic, Lutheran Social Services, and a large number of other providers of medical and social services.
